I agree to drop off and pick up my child at the scheduled times. Failure to do so will result in a $1.00 per minute charge. *

I will stay in constant communication with Redefined Teaching about my child’s progress in tutoring and in school.*

I give permission for my child to be photographed and/or recorded during activities associated with the tutoring program. I understand that these photographs and/or recordings may be used for marketing, *
